Section 1: Personal Information

  • Ethnicity: The client's ethnicity.
  • Phone: The client's primary phone number.
  • Date of Birth: The client's date of birth.
  • Languages Spoken: The languages the client speaks.
  • Email: The client's email address.
  • Address: The client's home address.

Section 2: Physical Characteristics

This section details the client's physical attributes.

  • Height: The client's height.
  • Weight: The client's weight.
  • Hair Color: The client's hair color.
  • Eye Color: The client's eye color.

Section 3: Emergency Contacts & Providers

This area lists crucial contacts and medical providers for the client. The information is organized into categories such as Guardian, Emergency, Guardian - POC, etc., with names, phone numbers, and other relevant details.

Section 4: Assigned Staff & Interdisciplinary Team

This section shows the staff and other professionals assigned to the client.

  • Assigned Staff: The name and contact information for the primary staff assigned to the client.
  • Interdisciplinary Team: A list of professionals, such as psychiatrists, medical liaisons, and behavioral specialists, who are part of the client's care team.

This section provides a quick overview of the client's health status.

  • Diagnosis: Lists any formal diagnoses the client has received.
  • Allergies: Lists any known allergies.
  • Diet: Specifies any dietary restrictions or requirements.
